Stomach Ulcer vs Gastritis: What’s the Difference and How Are They Treated?

Upper abdominal discomfort is one of the most common complaints people bring to a doctor and one of the most commonly self-diagnosed ones too. Someone has burning pain after eating, looks it up, and decides it’s either an ulcer or gastritis. The two terms get used almost interchangeably in everyday conversation, as though they’re different names for the same thing.
They’re not. They’re related conditions that share overlapping symptoms and some common causes, but they’re distinct in what’s actually happening to the stomach lining, how they’re diagnosed, and crucially, how they’re treated.
Understanding the difference matters because treating one when you actually have the other leads to incomplete management and symptoms that keep coming back.
The Stomach Lining What Both Conditions Have in Common
Both gastritis and stomach ulcers involve the stomach lining the mucosa, in different ways. To understand either condition, it helps to understand how the stomach normally protects itself.
The stomach produces hydrochloric acid, a powerful digestive fluid as a normal part of breaking down food. That acid would damage the stomach’s own lining if it weren’t for a protective layer of mucus that coats the inner surface. This mucus barrier keeps the acid where it belongs and prevents it from coming into contact with the delicate cells underneath.
When that protective barrier is compromised from infection, medication, stress, or other factors, acid starts damaging the lining. The nature and extent of that damage is what determines whether the result is gastritis or an ulcer.
What Is Gastritis?
Gastritis is inflammation of the stomach lining. The word itself breaks down simply “gastro” meaning stomach, “itis” meaning inflammation. The mucus barrier has been disrupted or weakened, acid and other irritants are reaching the lining, and the lining responds with inflammation.
Importantly, gastritis is inflammation without a specific open wound. The lining is irritated, swollen, and angry, but it hasn’t broken down into a discrete sore. Think of it like sunburned skin red, inflamed, sensitive, but intact.
Gastritis can be:
Acute – developing suddenly, often from a specific trigger. A heavy drinking session, a course of NSAIDs, a severe physical stress event, or a sudden bacterial infection. Acute gastritis causes fairly immediate symptoms and often resolves relatively quickly once the trigger is removed.
Chronic – developing gradually over time, often from ongoing H. pylori infection or long-term NSAID use. Chronic gastritis may cause mild or intermittent symptoms or sometimes no symptoms at all, while the inflammation quietly persists.
What Causes Gastritis?
H. pylori infection – this bacterium is the most common cause of chronic gastritis worldwide. H. pylori burrows into the mucus layer of the stomach lining, disrupts it, and causes ongoing inflammation. It’s transmitted through contaminated food and water and is more prevalent in areas with poor sanitation though it’s extremely common across all populations including urban India.
NSAIDs – non-steroidal anti-inflammatory drugs like ibuprofen, aspirin, diclofenac, and naproxen inhibit prostaglandins, which are chemicals that help maintain the stomach’s protective mucus layer. Regular or long-term use strips away this protection, leaving the lining vulnerable to acid damage.
Alcohol – directly irritates and damages the stomach lining, causing acute gastritis. Chronic heavy alcohol use leads to chronic gastritis.
Stress – physiological stress from severe illness, surgery, burns, or trauma can cause stress gastritis. The mechanism involves reduced blood flow to the stomach lining and changes in acid production.
Autoimmune gastritis – the immune system attacks the stomach’s own cells, destroying the glands that produce acid and a protein called intrinsic factor needed for vitamin B12 absorption. This type is associated with pernicious anaemia.
Other factors – bile reflux into the stomach, certain medications beyond NSAIDs, and radiation therapy can all cause gastritis.
Symptoms of Gastritis
The symptoms of gastritis are variable, which is part of what makes it tricky to distinguish from other upper GI conditions without investigation.
Common symptoms include:
- A burning, gnawing, or aching discomfort in the upper abdomen
- Nausea, often the most prominent symptom in acute gastritis
- Vomiting in more severe cases
- A feeling of fullness or bloating after small amounts of food
- Loss of appetite
- Belching
- Indigestion that doesn’t respond well to antacids
Some patients with chronic gastritis have very few symptoms or symptoms so mild and intermittent that they don’t seek help. This is particularly common with H. pylori-associated gastritis, where the infection can persist for years with only occasional mild discomfort.
The absence of prominent symptoms doesn’t mean the inflammation isn’t happening, chronic gastritis can cause progressive changes to the stomach lining over time even when symptoms are minimal.
What Is a Stomach Ulcer?
A stomach ulcer, more precisely called a gastric ulcer, and part of the broader category of peptic ulcers is a discrete open sore in the stomach lining. Unlike gastritis, where the lining is inflamed but intact, an ulcer represents a breach, the protective surface has worn away and the underlying tissue is exposed.
Think of the difference like this: gastritis is like skin that’s sunburned and inflamed. An ulcer is like a wound that’s broken through the skin.
Ulcers form when the balance between acid production and the stomach’s protective mechanisms tips far enough that acid erodes a specific area of the lining down to the tissue below. The result is an open sore that can range from a few millimetres to several centimetres in size.
Peptic ulcers can occur in the stomach (gastric ulcers) or in the first part of the small intestine, the duodenum (duodenal ulcers). They share similar causes but have some differences in symptom pattern.
What Causes Stomach Ulcers?
The causes overlap significantly with gastritis, which makes sense, since ulcers often develop in the context of ongoing gastritis.
H. pylori – the same bacterium that causes gastritis is responsible for the majority of peptic ulcers. The chronic inflammation it produces weakens the protective lining over time until a discrete sore forms. Eradicating H. pylori is one of the most important steps in treating H. pylori-associated ulcers.
NSAIDs – long-term or high-dose NSAID use is the second most common cause. The loss of prostaglandin-mediated mucosal protection allows acid to cause focused damage, particularly in the stomach.
Stress ulcers – severe physiological stress can cause acute ulceration, particularly in critically ill patients. These are distinct from the psychological stress that aggravates symptoms in people with existing ulcers.
Zollinger-Ellison syndrome – a rare condition where tumours in the pancreas or small intestine produce excess gastrin, driving massively elevated acid secretion that causes multiple, often severe ulcers.
Smoking – increases ulcer risk by reducing prostaglandin production and impairing mucosal defence.
Symptoms of a Stomach Ulcer
Ulcer symptoms share some overlap with gastritis, upper abdominal burning or gnawing pain, but with some distinguishing features:
- Burning or gnawing pain in the upper abdomen – typically more intense and more specifically located than the diffuse discomfort of gastritis
- Relationship to eating – this is one of the more helpful distinguishing features. Gastric ulcers typically cause pain that’s worse after eating, because food stimulates acid production and the acid contacts the sore. Duodenal ulcers often cause pain that temporarily improves after eating — food buffers the acid and then returns two to three hours later when the stomach is empty again
- Night pain – waking with upper abdominal pain in the early hours is particularly associated with duodenal ulcers
- Nausea and vomiting – can occur, particularly with gastric outlet obstruction if the ulcer is near the pylorus
- Loss of appetite and weight loss – more common with gastric ulcers
- Symptoms that come and go – ulcers often cause episodic symptoms with periods of relative improvement between flares
Complications of Ulcers
This is the key difference in terms of clinical significance. Gastritis is uncomfortable and needs treatment but gastric ulcers carry the risk of serious complications that gastritis generally doesn’t.
Bleeding – an ulcer can erode into a blood vessel, causing bleeding into the stomach. Mild bleeding produces dark, tarry stools (melaena) from digested blood. More significant bleeding causes vomiting of blood, either bright red or resembling coffee grounds. Upper GI bleeding from a peptic ulcer is a medical emergency.
Perforation – if an ulcer erodes all the way through the stomach wall, the contents of the stomach spill into the abdominal cavity. This causes sudden, severe abdominal pain, often described as the worst pain the person has experienced and is a surgical emergency. A perforated ulcer requires immediate surgery.
Gastric outlet obstruction – an ulcer near the pylorus (the outlet of the stomach into the duodenum) can cause scarring and narrowing that obstructs the passage of food. This causes persistent vomiting, particularly of undigested food.
Increased cancer risk – chronic gastric ulcers, particularly those associated with H. pylori, carry a modestly increased risk of gastric cancer over the long term. This is one reason that gastric ulcers are biopsied during endoscopy to check for malignant cells, whereas duodenal ulcers rarely are.
How Are They Diagnosed?
Both conditions benefit from proper investigation rather than assumed diagnosis and empirical treatment.
Blood tests – a full blood count checks for anaemia from bleeding. H. pylori can be detected through a blood antibody test, though this doesn’t distinguish active from past infection.
Stool antigen test for H. pylori – more accurate than blood serology for active infection. A stool sample is tested for H. pylori antigens.
Urea breath test – the patient drinks a solution containing labelled urea. If H. pylori is present, it breaks down the urea and the labelled carbon is detected in the breath. Highly accurate for active infection.
Upper GI endoscopy – the definitive investigation for both conditions. A thin, flexible camera is passed through the mouth into the oesophagus, stomach, and duodenum, giving a direct view of the lining. Gastritis appears as reddened, inflamed mucosa, sometimes with erosions. Ulcers appear as discrete craters in the lining.
During endoscopy, biopsies can be taken from the stomach lining, to confirm H. pylori histologically, to assess the degree of inflammation, and for gastric ulcers specifically, to check for malignant cells. This last point is important, a gastric ulcer that looks benign on endoscopy should still be biopsied and then re-examined after treatment to confirm it has healed. A cancer masquerading as a benign ulcer is an important diagnosis not to miss.
How Are They Treated?
This is where the difference between the two conditions becomes most practically significant, the treatment overlaps but isn’t identical.
Treating Gastritis
Treatment depends on the cause.
H. pylori eradication – for H. pylori-associated gastritis, the treatment is a course of combination therapy: two antibiotics alongside a proton pump inhibitor (PPI), taken for seven to fourteen days depending on the regimen. This “triple therapy” or “quadruple therapy” eradicates the infection in most cases, after which the gastritis resolves over time. Confirming eradication with a urea breath test four to six weeks after completing treatment is important, not all courses succeed first time.
Stopping or reducing NSAIDs – where NSAID use is the cause, stopping or switching to a safer alternative (with a PPI for gastric protection if NSAIDs can’t be avoided) is central to management.
Acid suppression – PPIs like omeprazole or pantoprazole or H2 blockers like famotidine reduce acid production, relieving symptoms and allowing the inflamed lining to recover. They’re used alongside H. pylori eradication or as the primary treatment where infection isn’t present.
Avoiding triggers – alcohol, smoking, spicy food, and caffeine worsen gastritis symptoms and slow healing. Dietary modification alongside medication improves outcomes.
Treating autoimmune gastritis – B12 supplementation for pernicious anaemia, regular monitoring for progression.
Treating Stomach Ulcers
The principles overlap with gastritis treatment, but with additional considerations.
H. pylori eradication – for H. pylori-positive ulcers, eradication is essential. Without it, ulcer recurrence rates after healing are very high, because the underlying cause hasn’t been addressed. Eradicating H. pylori dramatically reduces the risk of recurrence.
PPI therapy – ulcers need a longer course of acid suppression than gastritis to allow the sore to heal fully. Typically four to eight weeks, depending on the size and location of the ulcer. PPIs heal the majority of uncomplicated ulcers.
Stopping NSAIDs – where NSAIDs are the cause, stopping them is essential. If NSAIDs can’t be stopped for a patient who needs them for arthritis or cardiovascular reasons, a PPI should be taken alongside them for ongoing gastric protection.
Confirming healing – for gastric ulcers specifically, a repeat endoscopy after treatment is standard practice to confirm the ulcer has healed completely and to review biopsy results. This follow-up step is important and shouldn’t be skipped.
Managing complications – a bleeding ulcer is treated endoscopically when possible, injecting adrenaline around the bleeding point, applying thermal coagulation, or clipping the vessel during endoscopy. Surgery is reserved for bleeding that can’t be controlled endoscopically or for perforated ulcers, which require emergency surgical repair.
Lifestyle measures – stopping smoking, moderating alcohol, avoiding NSAIDs where possible, and eating smaller, more regular meals all support ulcer healing and reduce recurrence.

When Should You See a Doctor?
For mild, occasional upper abdominal discomfort, dietary modification and an antacid for a day or two is reasonable as a first step. But see a doctor if:
- Symptoms have been present for more than two weeks
- Pain is waking you from sleep
- Antacids are providing only temporary or no relief
- There’s nausea, vomiting, or loss of appetite alongside the pain
- Stools are dark or tarry, this needs same-day assessment
- There’s blood in vomit, go to the emergency department
- Unintentional weight loss is occurring alongside digestive symptoms
- You’ve been taking NSAIDs regularly for pain management
Final Thoughts
Gastritis and stomach ulcers are related but distinct conditions. They share causes and some symptoms but they differ in what’s happening to the stomach lining, the risk of serious complications, and the detail of how treatment needs to be approached.
